ABSTRACT

ABSTRACT Objective: To describe the population assisted in a genetics outpatient clinic, in a medium-sized town, with respect to diagnosis, type of inheritance, and local impact of genetic care. Methods: Medical records and genetic consultation forms from 2006 to 2018 were reviewed. The variables analyzed were age, sex, origin, current residence, reason for consultation, professional who requested evaluation, final diagnosis, additional exams and their results. Results: A total of 609 patients were seen, 65.9% aged 0 to 12 years. Genetic syndromes were suspected in 15.1%, and 11% presented developmental delay. Neurogenetic disorders stood out among adults. Mendelian inheritance was more prevalent (17.8%). Requests for genetic consultation have doubled in the last 5 years, with 44.4% due to suspected genetic syndrome. Conclusion: Genetic consultations have shown to be an important tool for inpatient care, reducing the waiting time to initiate treatment, attenuating potential associated costs, and guiding the families of patients. Outpatient care provided diagnosis and genetic counseling for users from the city and surrounding region, decreased costs and offered a training environment in medical genetics.


RESUMO Objetivo: Descrever a população atendida em um ambulatório de genética, em uma cidade de médio porte, em relação a diagnóstico, tipo de herança e impacto local do atendimento. Métodos: Foram revisados os prontuários e formulários do serviço de genética entre 2006 e 2018. As variáveis analisadas foram idade, sexo, procedência e atual residência, motivo de referência/consultoria, profissional que encaminhou, diagnóstico final, exames complementares e seus resultados. Resultados: Foram atendidos 609 pacientes, 65,9% com idade de zero a 12 anos. Houve suspeita de síndromes genéticas em 15,1%, e 11% apresentaram atraso no desenvolvimento. Distúrbios neurogenéticos destacaram-se entre os adultos. A herança mendeliana teve maior frequência (17,8%). As solicitações de consultorias genéticas duplicaram nos últimos 5 anos, sendo 44,4% por "suspeita de síndrome genética". Conclusão: As consultorias genéticas mostraram-se ferramenta importante em nível hospitalar, reduzindo o tempo de espera até a instituição terapêutica, minimizando custos potenciais associados e norteando os familiares. O atendimento ambulatorial ofereceu diagnóstico e aconselhamento genético para usuários da cidade e região, diminuiu custos, além de propiciar um ambiente de ensino em genética médica.

ABSTRACT Objective To evaluate the prevalence and factors associated with non-vaccination against influenza in the risk group. Methods A cross-sectional, population-based study, carried out in the city of Rio Grande (RS). The outcome was defined as belonging to risk groups and not having been vaccinated in the last 12 months. Demographic, socioeconomic, behavioral variables, and access for health services were analyzed. Results In this study, 680 individuals participated. The prevalence was 46.0% (95%CI: 41.8-50.3), ranging from 27.9% (elderly) to 81.8% (pregnant women). Young adults, single, intermediate socioeconomic bracket, smoker, with depressive symptoms, who did not perform physical activity and did not consult a physician in the last year, had a higher prevalence of non-vaccination. Conclusion Half of the sample was not vaccinated in the period. Due to the similarity of influenza-like illness and the coronavirus 2019 disease (COVID-19), increasing vaccination would minimize mortality and use of hospital beds due to influenza, optimizing the response of hospital capacity.


RESUMO Objetivo Avaliar a prevalência e os fatores associados à não vacinação contra influenza em grupos de risco. Métodos Estudo transversal, de base populacional, realizado em Rio Grande (RS). O desfecho foi definido como pertencer aos grupos de risco e não ter se vacinado nos últimos 12 meses. Foram analisadas variáveis demográficas, socioeconômicas, comportamentais e de acesso a serviços de saúde. Resultados Participaram 680 indivíduos. A prevalência foi de 46,0% (IC95%: 41,8-50,3), variando de 27,9% (idosos) a 81,8% (gestantes). Adultos jovens, solteiros, de nível econômico intermediário, tabagistas, com sintomas depressivos, que não praticavam atividade física e não consultaram um médico no último ano tiveram maior prevalência de não vacinação. Conclusão Metade da amostra não foi vacinada no período. Pela semelhança da síndrome gripal com a doença pelo coronavírus 2019 (COVID-19), aumentar a vacinação minimizaria a mortalidade e a utilização de leitos hospitalares devido à influenza, otimizando a resposta da capacidade hospitalar.

ABSTRACT

Objetivo: analisar a evolução do consumo de psicoestimulantes pelos acadêmicos de Medicina da Universidade Federal do Rio Grande (FURG) durante quatro anos. Métodos: foi realizado um estudo de painel com amostra de estudantes do primeiro ao quarto ano do curso de medicina, matriculados na instituição no período de 2015 a 2018. O estudo teve como desfecho o consumo de psicoestimulantes. Foram coletadas informações sobre o uso de cafeína, metilfenidato, piracetam, modafinil, bebidas energéticas, metilenodioximetanfetamina (ecstasy) e anfetaminas. O questionário foi composto de duas etapas. Na primeira, foram recolhidas informações demográficas, sobre hábitos e qualidade de vida. Na segunda, questionou-se sobre o consumo de substâncias estimulantes, abordando a frequência de uso, efeitos percebidos e a motivação para o consumo, assim como o início do consumo durante o curso. Resultados: a prevalência de uso dessas substâncias aumentou de 58% para 68% de 2015 a 2018. A proporção de acadêmicos que começaram a usar psicoestimulantes durante a faculdade, aumentou de 15% para 30%. Essa proporção aumentou conforme o ano do curso, passando de 25% no primeiro ano para 38% no quarto ano. Esse resultado foi atribuído, principalmente, ao uso de metilfenidato, cuja prevalência aumentou de 21% para 56% durante o período do estudo. Conclusões: o consumo de psicoestimulantes entre estudantes de medicina foi alto e o início de seu consumo durante a faculdade aumentou ao longo dos anos. Seu uso tem sido percebido como eficaz pela maioria dos usuários, o que pode dificultar o gerenciamento do uso indevido dessas substâncias.


Aims: To analyze the evolution of psychostimulants consumption by medical students of the Federal University of Rio Grande (FURG) during the period of four years. Methods: This was a panel study conducted with a sample of students from first to fourth year of medical training, enrolled on the institution on the period between 2015 and 2018. The main outcome of this investigation was the use of psychostimulants. We collected information about the consumption of caffeine, methylphenidate, modafinil, piracetam, energetic drinks, amphetamines and methylenedioxymethamphetamine (ecstasy). The questionnaire was composed by two sections. First, information about socioeconomic and demographic variables, daily habits and quality of life were collected. In the second part, participants were asked about use of stimulant substances, frequency of its use, perceived effects, their motivation for consumption, as well as the beginning of consumption during the course. Results: Prevalence of use of these substances have increased from 58% to 68% between 2015 and 2018. Proportion of students that had started to use psychostimulants during college increased from 15% to 30% in this period. That proportion increased according to the year of graduation, passing from 25% on the first year to 38% on the fourth year. This result may be attributed mostly to the use of methylphenidate, whose prevalence increased from 21% to 56% during the period of the study. Conclusions: The consumption of psychostimulants among medical students was high, and the beginning of its consumption during college has increased over the years. Its use has been perceived as effective by most users, which may hamper the management of the misuse of these substances.

Objetivo: objetivou-se comparar as prevalências e os fatores associados à realização de mamografia de acordo com dois critérios em vigência no Brasil. Métodos: estudo transversal, de base populacional, com mulheres entre 40 e 74 anos de Rio Grande, RS, Brasil. A cobertura de mamografia foi avaliada: 1) rastreamento anual para mulheres entre 40 e 74 anos; 2) rastreamento bienal para mulheres entre 50 e 69 anos. Os dados foram analisados por regressão de Poisson. Resultados: participaram do estudo 413 (critério 1) e 246 (critério 2) mulheres. A cobertura de mamografia pelo critério 1 foi de 49,4% (IC95% 43,8 a 55,0), oscilando de um quarto para as que não consultaram um médico no último ano até dois terços para aquelas com maior escolaridade. Segundo o critério 2, a cobertura foi de 65,5% (IC95% 59,2 a 71,7), indo de um terço entre as que não consultaram um médico no último ano a três quartos entre as obesas. Conclusões: a cobertura de mamografia diferiu conforme o critério empregado. Maior nível socioeconômico e ter consultado com médico no último ano foram os fatores mais associados, independentemente do critério.


Aims: we aimed to compare the prevalence and factors associated to mammography exam according to two Brazilian criteria. Methods: a cross-sectional, population-based study, with women between 40 and 74 years old from Rio Grande, RS, Brazil. The coverage of mammography was evaluated: 1) annual screening for women aged 40 to 74 years old; 2) biennial screening for women aged 50 to 69 years old. Data analyses were performed by Poisson regression. Results: it was included 413 (criterion 1) and 246 (criterion 2) women. The mammography coverage by criterion 1 was 49.4% (95%CI 43.8 to 55.0), ranging from a quarter for those who did not visit a doctor in the last year to two thirds for those with higher education level. Considering the criterion 2, the coverage was 65.5% (CI95% 59.2 to 71.7), ranging from one-third among those who did not visit a doctor in the last year to three-quarters among obese women. Conclusions: the mammography coverage differed according to the criterion considered. Higher socioeconomic status and having visiting a doctor in the last year were the most associated factors, regardless of the criterion.

ABSTRACT BACKGROUND: Population-wide screening for prostate cancer remains a controversial topic, given the need for an individualized approach to patients regarding the risks and benefits of prostate-specific antigen testing and digital rectal examination. OBJECTIVE: The aim of this study was to investigate the prevalence of, and factors associated with, prostate examination among men aged 45 or older. DESIGN AND SETTING: Cross-sectional population-based study developed in the city of Rio Grande (RS), Brazil. METHODS: The outcome of interest was a history of prostate examination (prostate-specific antigen testing or digital rectal examination). The following independent variables were analyzed: age group, skin color, marital status, schooling, economic level, leisure-time physical activity, smoking habits, excessive alcohol consumption, overweight, health insurance, visits to the doctor during the preceding year, hypertension and diabetes. After a two-stage sampling process, the final sample consisted of 281 male individuals. RESULTS: The prevalence of a history of prostate-specific antigen testing or digital rectal examination was 68.3% (95% confidence interval (CI): 62.2 to 74.5). The highest prevalence rates were observed among men aged 70 years or older (88%) and the lowest among smokers (36%). The following characteristics were found to be associated with the outcome: advanced age; marital status other than single; more schooling and higher economic status; practicing physical activity; non-smoking habits; overweight; having health insurance; and having visited a doctor during the preceding year. CONCLUSION: Approximately two thirds of the study population had been screened for prostate examination, mostly older individuals, with higher socioeconomic status and a healthier lifestyle.
